Transitional Care Navigator / East Valley

 

This position coordinates the patient's transition from an acute care facility to the post-acute setting or to the patient's home with community or other resources, based on the recommendations provided by the inpatient Care Today team. This position performs follow-up tasks identified in the inpatient discharge care plan for management of Banner patients across the health-care continuum.


Performs transfer of accurate, pertinent patient information between all appropriate entities of the post-acute care continuum. Essential component to the success of the Care Coordination Care Tomorrow team.

 

Required Qualifications;

- High school diploma/GED or equivalent working knowledge. The position requires a proficiency level typically achieved with 3 years of experience in health-care as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, Patient Care Tech, etc.

 

- Basic Life Support (BLS) Certification

 

On-Call is required.

 

Travel:

With our rapidly changing hospital environment and the need to manage patients through the continuum, quickly coordinate discharge/transitioning of hospital patients to a post-acute settings, this positin will float around the East Valley.  Can you work the AZ East side hospitals located below?

  • Banner Heart
  • Banner Ironwood
  • Banner Goldfield

Shift:

730a-6p, four 10-hour shifts, every other weekend, 1 major and 1 minor holiday

Job Summary

This position supports the smooth, timely, and coordinated client transition from acute care to alternative levels of care including but not limited to post-acute settings, community services, or physician offices as directed by the care coordination team. This position performs follow-up tasks identified in the inpatient discharge care plan for management of Banner patients across the healthcare continuum.

 

Essential Functions

  • Works with the patient to coordinate transition into or out of a care setting in order to obtain appropriate services and benefits as directed by the care coordination team. This may include faxing information, entering referrals or tasking other departments or consultants, arranging authorization and transportation, arranging durable medical equipment (DME), coordinating home health care, confirming arrangements, obtaining clothing, making physician or outpatient appointments, obtaining test results, delivering equipment from DME closet, and other patient related duties as designated.

 

  • Performs transfer of accurate, pertinent patient information between all appropriate entities of the acute and post-acute care continuum. Performs follow-up calls to patients and providers regarding their experience with Banner; documenting findings and reporting any concerns or dissatisfaction to leadership for follow-up, issue resolution and data collection.

 

  • Participates in performance improvement projects, Banner initiatives and performs data collection for measurement of projects as assigned.

 

  • Documents all interventions in the patient medical record both timely and accurately including all elements of the discharge plan. Performs transfer of accurate, pertinent patient information between all appropriate entities of the post-acute care continuum.

 

  • Works collaboratively with team members; promotes collaborative relationships with vendors, community and referral resources.

 

  • May perform tasks such as securing community resources/information or other tasks.

 

 

Minimum Qualifications

High school diploma/GED or equivalent working knowledge.

The position requires a proficiency level typically achieved with 3 years experience in healthcare as a Nursing Asst, Medical Asst, Health Unit Coordinator, Patient Care Tech, etc. Must demonstrate effective communication and customer service skills, human relation skills and time management skills with flexibility in responding to multiple demands. In acute care must be able to work flexible hours and work after hours/weekends on rotation.BLS is required for acute-care settings where direct patient care is provided. (BLS is not required for employees working in the Insurance Division.)
