    Job Summary

    This position provides comprehensive care coordination for patients as assigned. Provides clinical assessment, psychoeducation, and psychotherapy services for defined patient population. Position provides treatment in one or more of the following areas: substance abuse, emotional and behavioral issues, eating disorders and other mental disorders as outlined in the "Diagnostic and Statistical Manual of Mental Disorders" (DSM).

     

    Essential Functions

    • Manages individual patients across the health care continuum to achieve the optimal clinical, financial, operational, and satisfaction outcomes.

     

    • Provides assessment, direct psychoeducation, and direct psychotherapy to individuals, families, and groups. Coordinates all aspects of patient care from admission to discharge, interacting with the physician and other members of the clinical treatment team to provide quality patient care.

     

    • Acts in a leadership function with process improvement activities for populations of patients to achieve the optimal clinical, financial, operational, and satisfaction outcomes. Collaboratively develops and manages the interdisciplinary patient discharge plan. Effectively communicates the plan across the continuum of care. May supervise other staff.

     

    • Provides psychoeducation and orientation for patients, families and others to assigned treatment program to aid in therapeutic process. Collaborates with the patient in the development of a written treatment plan. Includes patient and family involvement in assessment, treatment, education and discharge planning.

     

    • Establishes and promotes a collaborative relationship with physicians, payers, and other members of the health care team. Collects and communicates pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.

     

    • Monitors patients on an ongoing basis to continually assess needs and facilitate treatment and interventions. Documents clinical assessment, diagnosis, treatment plan, diagnostic summaries, and progress charting in a timely manner to assure comprehensive and quality patient care.

     

    • If working with involuntary patients, collaborates and coordinates Title 36 related issues as well as changes in patients legal status with hospital staff, County Attorneys Office, and outpatient agencies. May Transport patient to mental health hearings and disseminate court outcome.

     

     

    Minimum Qualifications

    Requires masters degree in social work or related field or equivalent experience.

    Requires a proficiency level typically achieved with 5 years clinical experience and as required by state law, a licensure in at least one of the following categories: social work, professional counseling or marriage and family. (Certification requires two years post graduate experience in a mental health setting or equivalent combination of education and documented supervised clinical experience.) Positions providing direct services to substance abuse program patients must also possess board licensure in addictions as required by state.

    Must have a working knowledge of care management, hospital and community resources. Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills. Must demonstrate ability to work effectively in an interdisciplinary team format.

    Employees working at Banner Behavioral Health Hospital must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment

    Preferred Qualifications

    Additional related education and/or experience preferred.
